MOORHEAD — Clay County residents who received a COVID-19 vaccine from an out-of-state pharmacy or the Fargo Veterans Affairs system are asked to inform Clay County Public Health of their vaccination status.

In addition to calling 218-299-7204, residents can use an online form to provide Clay County Public Health with their vaccination information. The required information is found on one's vaccine card.

Clay County Public Health is requesting the information so Minnesota residents can be recorded in the state's vaccine registry. The information can be helpful for future use because it will store the information for reference, the county said. It also helps public health officials know how many Clay County residents have been vaccinated.
