Beginning Aug. 1, Innovis Health in Fargo will implement specific procedures to verify patient information and to protect patients from identity theft.
Innovis requests that all patients be prepared to provide one of the following forms of identification throughout the registration and payment processes: driver's license, student ID, military ID or a passport.
